As a new participant, please complete the online Health Risk Assessment (HRA). The HRA will provide your current health status and estimate your level of health risk.
To complete your Health Risk Assessment (HRA), at the bottom of this page, click on Health Risk Assessment. Once completed, print and submit the document. It will automatically be transmitted to one of our wellness health coaches. Once we receive your HRA, we will mail you a lab slip that can be taken to any LabCorp draw station. If you have difficulty finding a lab to draw your Health Risk Lab Panel, please contact us at: wellness@specialtyhealth.com, 888-992-9974 or 775-398-3605.
